Cape Cod Healthcare Seeks Community Benefits Grant Applicants

(Hyannis, MA – July 15, 2026) – Cape Cod Healthcare (CCHC) is pleased to release its 2027 Annual Strategic Grant Request for Proposals (RFP). The 2027 RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County. Organizations are invited to submit proposals for programs or projects that improve the physical and/or mental health of Barnstable County residents. The CCHC Community Benefits grants program is open to all nonprofit community organizations operating in Barnstable County, which have programs aligned with CCHC Community Benefits priorities.

Successful proposals will:

  • Address one or more of the priorities listed in CCHC’s 2026-2028 Community Health Needs Assessment, including healthcare access, behavioral health and housing, with an emphasis on health equity and economic hardship.
  • Explain how the proposed program improves health
  • Promote and address health equity
  • Describe how the project impacts residents of Barnstable County

Community Benefits will host two grant information sessions:

  • Wednesday, Aug. 5, at 1 p.m. In person at the Cape Cod Healthcare Foundation, 4 Bayview St., West Yarmouth.
  • Thursday, Aug. 6, at 10 a.m. Virtual via Microsoft Teams; link to be provided upon registration.

These sessions will provide an overview of Cape Cod Healthcare’s annual strategic grant process and are intended for community members who are new to Cape Cod Healthcare’s grant-making process. All are welcome, and registration is required by emailing Kriste Hancock, Development and Community Benefits Associate, at kahancock@capecodhealth.org. Those unable to attend may contact Kriste Hancock to schedule an appointment for assistance. Attendance at an information session is not required to submit a grant application.

Cape Cod Healthcare Community Benefits will award up to $300,000 total in FY27 Annual Strategic Grants. Grants are for one year only and the maximum grant award is $30,000. The deadline for submission of strategic grant proposals is Sept. 13, 2026. Applicants will be notified of the awards by November 2026.

The CCHC Community Benefits program promotes the Cape Cod Healthcare mission to coordinate and deliver the highest-quality, accessible health services to Cape Cod residents and visitors.
