Applicants Sought For Cape Cod Healthcare Community Health Benefits Grants

Cape Cod Healthcare (CCHC) is seeking applicants for its 2024 Community Health Benefits Grants.

Organizations are invited to submit proposals for programs or projects that improve residents’ physical and/or behavioral health. The CCHC Community Benefits grants program is open to all nonprofit community organizations operating in Barnstable County, which have programs aligned with CCHC Community Benefits priorities.

Cape Cod Healthcare Community Benefits will award up to $300,000 total in FY24 Annual Strategic Grants. Grants are for one year only and the maximum grant award is $30,000. The deadline for submission of strategic grant proposals is Sept. 15, 2023. Applicants will be notified of the awards by November 2023.

Successful proposals will:

  • Address one or more of the priorities listed in CCHC’s 2023-2025 Community Health Needs Assessment
  • Explain how the proposed program improves health
  • Promote and address health equity
  • Describe how the project impacts residents of Barnstable County

    Community Benefits will host two grant information sessions at the Cape Cod Healthcare Foundation, 4 Bay View Street, West Yarmouth on Monday, Aug. 14, from 2 to 4 p.m. and Tuesday, Aug. 15, from 10 a.m. to noon.

These sessions will provide an overview of Cape Cod Healthcare’s Annual Strategic Grant process and are intended for community members who are new to Cape Cod Healthcare’s grant-making process. All are welcome and registration is required by emailing communitybenefits@capecodhealth.org. If you cannot make one of these information sessions, reach out to communitybenefits@capecodhealth.org to make an appointment for assistance. Note: You do not have to attend a session to submit a grant application.
